Our Mission
Prior to admission, a nurse will assess your loved one to determine if Park Place can meet the needs to properly care for them.
Admission
Admission Requirements
Park Place will work together with the family and the doctor’s office to obtain the items needed.
Admission requires approval from a physician who will need to provide the following:
Signed History & Physical
A signed history & current physical (physical must have been completed within the last six months)
Signed Order for Care
A signed order stating that the potential resident requires assisted care living
Signed Medication
A signed order for medication (both prescription and over-the-counter.)
Park Place Staff members are happy to assist with any arrangements necessary to help make a smooth transition for your loved one.
